Privacy Rights Request

What you can ask for

• Know / access: a copy of the personal and health information we hold about you.
• Confirm sharing: a list of any third parties or affiliates we have shared your health data with.
• Correct: fix inaccurate information.
• Delete: delete your information and, if you wish, your account.
• Portability: a portable copy of your data.
• Withdraw consent: stop our collection or sharing of your health data.
• Opt out of sale or sharing for advertising: we do not sell or share your data for advertising, so there is nothing to opt out of, but you may confirm this.

How to submit a request

Submit the form below, or email privacy@spotyellow.com with the subject "Privacy Request."

Request form fields:

• Your name
• The email address linked to your Yellow account
• Your state or country of residence
• The right you want to exercise (choose from the list above)
• Any details that help us locate your data
• Whether you are submitting on someone's behalf as an authorised agent (if so, we will ask for proof of authorisation)

What happens next

1. Acknowledgement. We confirm we received your request.
2. Identity verification. To protect your data, we verify your identity before acting, usually by confirming control of the account email. We use the information only to handle your request.
3. Response. We respond within 45 days. If we need more time, we may extend once by a further 45 days and will tell you why.
4. Appeal. If we deny your request, we will explain why and how to appeal. To appeal, reply to our decision or email privacy@spotyellow.com with "Appeal" in the subject line.

We will not discriminate against you for exercising any privacy right.

Authorised agents

You may use an authorised agent to submit a request. We will ask the agent for proof that you authorised them, and we may ask you to confirm your identity directly.
